1) We should put the object in the active sentence as subject in passive. In this question...there are two objects ie. Direct (book) and indirect (him) and any of these two objects can be put as subject in passive sentence. 2) Next we have to change the verb of the sentence...if it is given in past we should change it into `` was or were + past participle of the verb(in this question `was given') 3) Finally the passive voice verb has to followed by the word `by' and the doer. You can refer the rules of changing active sentence to Passive

There are two objects here.. anything can be used in the beginning of the sentence.
