Doubt regarding Exam formalities

Is it required to send our exam registration form signed by a CA or any Gazetted officer be sent to ICAI for May 2021 attempt? { 'Cause last time (Nov 2020) it was not required due to Covid situation} If yes, can you please give the address of ICAI Head office.

In case u haven't got ur photo and sign..the address to which the form has to be sent
