how can i surrender my permanent exemption in sept 2025 attempt paper

jay ksir can you please tell the prescribed form in which we have to submit affidavit
Stamp paper of 10/20 AFFIDAVIT I, [Your Full Name], son/daughter of [Father's/Mother’s Name], residing at [Full Residential Address], do hereby solemnly affirm and declare as under: That I am a student registered with The Institute of Chartered Accountants of India (ICAI) bearing Student Registration Number: [Your Reg. No.]. That I appeared for the [CA Final / Intermediate / ] Examination held in [Month & Year] and was granted exemption in Paper No. ___: [Name of the Paper], having secured [Marks obtained] marks, as per the provisions of ICAI. I subsequently availed Permanent Exemption in the Subject. That I do not wish to avail the said exemption in the upcoming examination to be held in [Month & Year], and I hereby voluntarily surrender the Permanent exemption granted to me in the above-mentioned paper. That I understand that the exemption once surrendered cannot be claimed back under any circumstances for the said attempt. That I am making this affidavit voluntarily, out of my own free will, and with full understanding of the implications thereof. That I undertake not to make any claim in future regarding the said exemption for the said attempt.
