Whether both the "Holder of the share" and the "Person holding beneficial interest " are required to submit the declaration (i.e.) FORM MGT 4 & 5 or whether it is sufficient if any one of them submits the declaration?

No both registered owner and beneficiary require to submit declaration to the company in MGT 4 and MGT 5 respectively.
