Here, it is said that the operational head succeeded, which means that the strategy has been formulated correctly. But, still the desired competitive position, i.e., the end result, is not achieved, which means that there is a problem in the implementation. So, there has been a fault in the strategy implementation, and hence, it is the answer for this question.
